  • Are you out of Contract, Is the service in your name. Only Port if “Yes”.

  • Account Number. If Prepaid “Their date of Birth”, If account or Postpaid Number is on their Bill.

  • Type of service.  Postpaid = Account (Gets and Pays a Bill) Prepaid = Buys Credit when required (Like Load)

  • Service Provider Who sends them a Bill each mth. Who takes their customer service?

  • Network What companies Network are they using?

  • Technology GSM = Digital Phone (What we sell),
    CDMA = Extended Coverage (Better Country area),
    G3 = High Speed Data Phone (Video Phone).

  • Is the Service in your name. If not, put he owner of the Phones details here.
